Two poles: controlled traction of the spine

/ Method

What it is

What it is

You lie on the table, secured with straps. The device pulls according to a set program: the force builds and eases in waves rather than staying constant.

A detail worth knowing

The goal is to briefly reduce pressure inside the disc and give space to a compressed nerve root. This is not “putting things back in place” and not manual therapy.

What it feels like

A feeling of gentle pulling in the lower back or neck, which builds and then lets go.

No pain

It should not hurt. If the pain increases, we stop and review the program.

  • we set the intensity together — if it is unpleasant, we lower it
  • right after the session you can work with your body

/ How long

How long
it takes

Traction is not a one-off manipulation. The first relief often comes right away, the rest builds up over the course.

10–25 minutes

The session itself

You lie down and the device pulls according to the program the therapist set. No preparation needed.

Same day

First relief

Relief of the nerve root is often felt after the very first procedure.

Every other day

The rhythm of the course

In the acute phase — daily for the first week, then every other day. Less than 1–2 times a week makes no sense.

About 10 sessions

The full course

That is usually what it takes for the changes to hold. We review the exact number and the settings as we go.

After a session we do not load the back with exercises that day: the tissue needs to settle into its new position.

/ When we prescribe it

When traction makes sense

  • pain radiating into the leg or arm
  • protrusions and herniations with nerve root symptoms
  • a feeling of being “pinched” in the lower back
  • stiffness that does not let go after sleep

Traction gives you a window without pain. What you do in that window decides the result. When it does not suit If the pain is local, muscular and does not radiate, there is nothing to pull. We work differently.

/ Contraindications

When traction is not used

  • osteoporosis
  • a fresh vertebral fracture
  • segment instability, spondylolisthesis
  • the state after spinal surgery, including with metal hardware — only by agreement with the surgeon
  • severe pain in the acute phase
  • aortic aneurysm, serious cardiac conditions
  • pregnancy
  • cancer
  • The final decision is up to the physical therapist. At the assessment we ask about everything that could be a contraindication.

Tell us about it at the assessment. It changes the choice of method — we will pick another one that works in your case.

/ Questions and answers

Frequent questions

Is this the same as “putting a vertebra back”?+
No. We do not put anything back and we do not twist anything. The device creates smooth, dosed traction according to a program the therapist sets.
Is it safe with a herniated disc?+
With no contraindications — yes, and it is exactly with nerve root pain that the method fits best. But an examination comes first: with instability or osteoporosis traction is not allowed.
Can it make things worse?+
If the pain increases during a session, the procedure is stopped. That is why we keep asking how it feels rather than leaving you alone on the device.
How many sessions?+
Usually about ten, every other day, and in the acute phase daily for the first week. This is a benchmark: some people need fewer, some need longer — it depends on what is compressed and how quickly it releases. We review the settings as we go.
